Why Synergy Care?
Synergy Care helps rehabilitate people and businesses by improving patient outcomes while reducing compliance risks.
A Strategic Approach to Therapy
Skilled nursing facilities and long-term care organizations that want to deliver comprehensive therapeutic care while maintaining compliance and financial stability must hire highly-trained staff. Synergy Care provides the staff you need – who are familiar with the latest care techniques, coding requirements and payment methodologies – just to name a few. Identifying and retaining this caliber of talent can be difficult – if not impossible – for independent organizations, especially amid staffing shortages and the shift to PDPM.
Our clients eliminate the financial risk associated with in-house therapy, as well as the challenge of recruiting and retaining quality therapists. Synergy Care also assumes responsibility for compliance issues and allows for the indemnification of therapy denials. By partnering with us, you can keep your independence while enriching your clinical offerings and helping your organization stand out in the community as one committed to unfailing excellence.

Continuous Improvement Drives Business Forward
Our staff is passionate about continuous performance improvement. Here are some of the areas we focus on to ensure our clients are well-positioned for long-term success:
• Reducing hospital readmissions
• Improving quality outcomes
• Following clinical guidelines and care pathways
• Discharge planning at the start of care
• Collaborative care transitions
• Thorough patient evaluations and regular baseline testing
• Proactive chronic care management and early intervention
• Safe discharge assessments
• Multidisciplinary collaboration

Synergy Care Improves Therapy Engagement
Izzy, Synergy Care’s therapy dog, makes therapy more fun and engaging for the patients we serve. Her best trick is stretching when told to do “yoga” when she works with patients to understand the importance of preparing for therapy sessions.
While the runt of her litter, Izzy is now the biggest of her brothers and sisters. This interesting fact lets people know that we can all strive for growth and improvement – even if it seems hard to overcome recovery challenges.
Rest is essential for rehabilitation, so Izzy shows how it should be done with frequent naps – snoring in her dog bed in Mom’s office while Mom (Natalie Rabalais, Regional Manager) works. Izzy’s real mom and dad, Inez & Knox, are owned by another Synergy Care employee, Montana Adkinson.
We try to make days special at Synergy Care, and a little pampering goes a long way with Izzy! She loves being dried off with a blow dryer. If she hears one turn on, she runs straight to it.
Izzy has been a wonderful addition to our Synergy Care team. She loves all the attention she has been getting at our facilities, and we look forward to introducing her to you.
